Human Error–Induced Risk in Reinforced Concrete Bridge Engineering

Throughout the last century and in recent years, several bridge failures have taken place worldwide. Recent studies uncovered that primary cause of these collapses was human errors design, construction, operation phases. Regardless this finding, there is still a considerable gap between information known risk they represent for structural safety. Aiming better understanding errors, an identification procedure qualitative assessment such considering risk-based indicators (probability occurrence consequence) performed. Several brainstorming meetings with design construction experts led to 49 relevant which were listed further evaluation on survey. Much more important than identifying assessing those pose greater threat Using decision-making tool (analytical hierarchy process) process all collected survey, ranked according indicators. Furthermore, performed, allowing denoting higher safety, experts’ opinions.
